Individual Giving
When making a contribution, your donation will be directed to the General Foundation Fund unless you designate the name of a legacy fund, program, or fundraising challenge in the dropdown menu.
Currently active programs include:
- ASID Foundation General Fund
- Advocate by Design (AxD) Fund (*this is not tax deductible)
- ASID Chapter Challenge
- Gary Wheeler NCIDQ Fund
- Giving Tuesday
- Marty Rayle Fund
- Rocky La Fleur Fund
Currently with our system, donations made online with a credit card will require you to sign up for an ASID account unless you already have one. If you do not have an account you can create one without becoming a paying ASID member. When making a donation online by credit card, you will automatically receive a donation receipt by email.
Donations by check can be sent to the following address. Please make checks payable to “ASID Foundation” and on the top of the check make a note if there is a named fund to which you wish to direct this. Donation receipts will be mailed after donations are processed. Mail your check to:

Legacy Giving
Planned Giving
The ASID Foundation has established The Legacy Society to provide a number of Planned Giving methods. Be sure to include the ASID Foundation in your estate/financial planning. Some of the benefits of proper planning are:
- Bypassing capital gains tax on appreciated property
- Increasing your current income
- Reducing current income taxes
- Receiving a charitable income tax deduction
- Reducing federal estate taxes
- Conserving future assets for your loved ones

Tributes & Memorials
Tribute and memorial gifts offer a meaningful way to honor a loved one while supporting a cause close to your heart. A tribute gift celebrates special occasions such as birthdays, anniversaries, or achievements, while a memorial gift serves as a lasting remembrance of someone who has passed. These contributions create a legacy, helping to fund vital programs and initiatives. Donors may also request acknowledgment cards to inform families or honorees of their thoughtful gift. At the ASID Foundation, there are two types of tribute gifts:
Tribute gifts
Tribute gifts recognize a person on a special occasion, such as a birthday, wedding, or anniversary.
Memorial gifts
If you would like your loved one, or their family, to receive a special acknowledgment that recognizes the honoree and the donation, please contact one of our Trustees or e-mail us at foundation@asid.org.
Matching Gifts
A way to double your gift
Individual gifts may also be eligible for a matching contribution from a donor’s company. Many corporations and businesses nationwide offer matching gift programs. Employees, retirees and/or employees’ spouses all have the opportunity to maximize their contribution through a company match.
By taking advantage of a company’s matching gift benefit, you may be able to double the amount of your gift to the ASID Foundation. Employer matching gifts are a great way for you to maximize your personal contributions to the Foundation and to increase the impact of your gift.
Please contact your human resources or matching gifts department to see if matching gifts are offered at your company. If your company is eligible, request a matching gift form from your employer and send the completed form with your payment or receipt of payment and we will do the rest. If your employer does not participate, please contact your human resources or matching gifts department to see if you can establish a matching gifts program with the ASID Foundation.
Follow these 3 simple steps to apply for a Matching Gift:
- Obtain a matching gift form and program guidelines from your human resources or matching gifts department.
- Fill in the Employee section of the form and send it to The ASID Foundation with your donation, or if you’ve made your contribution online simply complete the form, noting your donation, and mail it to:
